The Triage Nurse, HCH, is part of the HomeCare & Hospice interdisciplinary team, supporting general care processes, clinicians, patients, and their caregivers in the home setting. Provides home health and hospice patients with telephonic access to highly skilled and trained registered nurses who accurately answer questions and address health concerns. Provides clinical support as an extension of the patient care team when receiving patient related calls and when handling all patient clinical needs. The Triage Nurse, HCH focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the health care system, and evaluating patient outcomes. Applies critical reasoning and astute clinical judgement to conduct patient telephone assessment, utilizing nursing skills to determine problems, intervening according to the patient’s needs, and providing instructions to patient/caregiver. Responsible for evaluating and assessing the home health care needs of patients based on telephonic evaluation of patient reported signs and symptoms. Delivers, manages, and coordinates care and services within the domain of home health and hospice care and ensures continuity of care through communication with other members of the patient’s health care team.

Position:

Triage Nurse HCH

Shift:

full time Monday-Friday; day shift (8:30 am to 5:00 pm) + 1 weekend per month

Experience:

1. Recent clinical experience in critical care, emergency department, home health, or physician practice nursing is required.

2. Telephone triage experience of at least 1 year is strongly preferred.

Education:

Graduation from an accredited School of Nursing required.

Bachelor of Science in Nursing (BSN) strongly preferred.

Licensures/Certifications:

1. Active Pennsylvania Registered Nursing license required.

2. BLS (American Heart Assoc/American Red Cross) required.

3. Nursing specialty certification preferred.
