Take pride in transforming primary care into "what patients want it to be" through care coordination and communication. Responsible for the development and growth of the Palliative Care /Pain Management Program. Functions as a consultant, clinician and mentor in the delivery and evaluation of palliative care and pain management services to hospitalized patients. Performs patient assessments and develops interdisciplinary plans. Collaborates with interdisciplinary team as necessary. Provides educational programs related to palliative care.

Position:

Palliative Care Nurse Coordinator

Shift:

Day (Monday through Friday 8:30AM to 5PM) No holidays or weekends.

Experience:

Three - five years nursing experience in critical care, oncology, palliative care or hospice care Excellent communication and customer service skills for all aspects of customer (i.e. patient) interactions. Knowledge of word-processing and familiarity with Microsoft office required

Education:

Graduate of approved School of Nursing required. Bachelor's Degree preferred, master's degree preferred. Ability to understand written and verbal instructions in English sufficiently to perform job duties.

Licensures/Certifications:

1. Current Professional Nursing Licensure in Pennsylvania OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.

2. Provider BLS (American Heart Assoc/American Red Cross) required.; Certification in Palliative Care preferred

Pay Range: $99,070.40 - $154,544.00
