The Advanced Heart Failure Nurse Navigator supports patients receiving care through the IV Diuretic Clinic by providing clinical assessment, care coordination, and patient education. Responsibilities include conducting telephone outreach to monitor weight changes, fluid retention, symptoms, and medication adherence; triaging patient concerns; obtaining labs; placing peripheral IVs for diuretic administration; and monitoring patient response to treatment. The Nurse Navigator also performs follow-up calls within 24 hours of treatment, communicates findings with providers, coordinates ongoing care, and assists with management of the clinic In Basket, including patient messages, provider-assigned tasks, lab results, and other care coordination activities. This role works closely with the multidisciplinary heart failure team to improve symptom management, support timely interventions, and help prevent avoidable hospitalizations.

Position-Specific Benefits include:

Nurse Navigator

Shift:

Days, 16 hours per week.

Qualifications:

1) Current Registered Nurse (RN) license.

2) Experience in cardiology, heart failure management, ambulatory care, infusion therapy, or care coordination preferred.

3) Proficiency in venipuncture, specimen collection, and peripheral IV insertion

4) Strong clinical assessment, patient education, communication, and care coordination skills.

Essential Accountabilities:

  • Conduct telephone outreach to patients referred to the IV Diuretic Clinic to assess symptoms, weight trends, fluid retention, edema, shortness of breath, and medication adherence.
  • Draw laboratory specimens including STAT laboratory testing and establish and maintain peripheral intravenous access for administration of IV diuretic therapy.
  • Conduct follow-up calls within 24 hours of IV diuretic treatment to evaluate symptom improvement, weight changes, urine output, medication adherence, and overall response to therapy; communicate findings with patient’s cardiologists, advanced practice providers, primary care providers, and other members of the healthcare team.
  • Assist with management of the clinic In Basket, including patient messages, telephone encounters, refill requests, provider-assigned tasks, lab result notifications, and care coordination activities.
  • Education:

    Graduate of an accredited ASN or BSN nursing program, required. Graduate degree is preferred.

    Licensures/Certifications:

    1. Current Pennsylvania RN license.

    2. Provider BLS certification from the American Heart Association or American Red Cross required.
