The ED Care Coordination Assistant will provide a comprehensive assessment and appropriate interventions for Emergency Department (ED) patients with identified Social Determinants of Health needs. This role will support patients who lack access to primary care, durable medical equipment, dialysis, medications, food, or financial resources. They will collaborate with the ED Care Manager and clinical team to ensure suitable utilization of appropriate services as well as connections to community resources prior to discharge. The ED Care Coordination Assistant will work closely with the ED Social Worker and will interface with clinical staff, physicians, and external agencies to arrange for appropriate community-based services.

Position:

ED Care Coordination Assistant

Shift:

Monday - Friday; 8:00 am - 4:30 OR 3:00 pm - 11:00 pm

Experience:

1. Min of 1 year experience in a healthcare setting.

2. Experience managing high-utilizer patients and addressing Social Determinants of Health (SDOH) strongly preferred.

3. Prior exposure to emergency department (ED) care coordination and/or ambulatory care settings preferred.

4. Familiarity with care transitions, discharge planning, and community resource coordination preferred.

Education:

Bachelor's in SW, Healthcare Administration, Public Health, Nursing or related field

Licensures/Certifications:

Care Coordination or Case Management certification preferred

Employee Status: Regular

Pay Range: $28.89 - $44.77

Job Grade: 210
