By gathering and verifying medical, demographic, financial, and social information, this role supports accurate admission decisions and prepares the care team for each patient’s needs. The coordinator serves as the central point of communication—managing referrals, bed placement, census updates, and coordination with physicians, nursing units, insurance representatives, and families. Their work maintains patient flow, supports clinical readiness, and ensures a smooth, informed transition into inpatient rehabilitation.
